Pre-Participation Physical Evaluation
REQUIREMENT:
All student athletes are required to submit a sport's physical (dated after July 1, 2012) to the respective head coach on the FIRST day of TRYOUTS.
ALL sport’s physicals for the 2012-2013 school year must be dated after July 1 2012. Sport’s physicals are valid the first day of school until the last day of school. A NEW PHYSICAL FORM MUST BE COMPLETED EVERY SCHOOL YEAR.
